The occupational cardiology is a new medical discipline that connects cardiology and the preventive occupational medicine. It has the following primary objectives:

- to find a better understanding of the job stress relation to the aggravation of arterial hypertension and of other cardiovascular anomalies;

- to develop a clinic exam that will evaluate and interpret the work history, and will discover the the role of stress in the apparition and the aggravation of cardiovascular diseases.

- to establish, test and validate algorithms and recommendations for the diagnostic and the management of the arterial hypertension cause by job stress along with other cardiovascular diseases that are affected by stress;

- to promote a cooperation between the cardiologists and specialists in the work medicine, occupational psychology, epidemiologists, manager, labor unions and employees from different activity sectors, public or private.

- to assure the legislative political support in the environment of the 21 century medicine characterized by the increasing number of medical doctors and psychologists hat participates and design the implementation of changes in workplace conditions on the base of clinical findings and monitoring effects of the current management of employees.

Till several years ago, many of the causes of the arterial hypertension were not known (the only acknowledged risk factors were obesity and excessive consume of salt) and as a result the treatment was based on symptoms and had a long duration.

 

Recent researches have identified the job related stress as the main cause of the apparition and aggravation of arterial hypertension. In order to understand the work related stress and the risks produced by it in the area of physical and mental health, Robert Karasek proposed the Job Strain Model that has at its base two major coordinates: the requirements of the workplace and the competence, along with the liberty allowed in taking decisions or in executing tasks.

Psychological tension at the workplace reaches its maximum in the situation where there are very high requirements and expectations, but there is a minimal level of liberty allowed in taking decisions or executing normal tasks.

Intense and long term psychological tension at the workplace is leading to the long term activation of the sympathetic nervous system. The consequences are apparition, and on a long term presence, aggravation of the metabolic syndrome (a series of symptoms that are often signalized in parallel with: obesity, hyperglycemia, hyper insulin , and a high profile of lipids).

The mechanism through which previous causes of illnesses become present effects of an individual's capability of cooperating with daily stress agents is confirmed. The World Health Organization have included this on its white paper strategy written in 1978, called Healthy living for everybody till the year 2000, in the objective no. 16 a healthy living:

The overweight, smoking, alcoholism, drug dependence and a lack of physical exercises are the indications of an individual's incapacity of coping with daily stress agents.

The above mentioned model can be applied also in the family or community life.

 

In this situation the psychological tension, which also generated arterial hypertension, appears in the situation in which a member of a family or a community has lots of task to accomplish, but the capacity of doing them is too little because of a lack of financial, material and time resources and is accentuated by a lack of a support network that was supposed to be composed by other members of the family or of the community.

The cardiovascular illnesses along with the accompanying symptoms of the metabolic syndrome have a fast evolution at the persons that are dealing with the psychological tension at the workplace and experience the same tension in the middle of the family or of the community.

The order no. 761/30.10.2001 regarding the approval of the methodology involved in the medical exam done prior to employment, the adaptation medical exam, the periodical exam check and in the back-to-work medical examination.

 

Art. 2. The medical exams specified in Art. 1 represent the active supervision of the employees health in relation to the professional factors involved in their job / workplace.

Art. 3. In the present regulations the professional factors are physical, chemical and biological factors that are present in the work environment that act independently or correlated, along with any relevant element from the activities involved for which there are scientific proof of a significant positive association between the exposure and the reversible or irreversible modification of the structure or the functionality of an body organ or system, or of the organism as a whole.


Art. 4.

The employers from the public, private or cooperative sector, including those that are funded by foreign capital, are obliged to assure the monitoring of their employees in relation to the characteristics of their job position, by providing workplace medicine services.

Art. 6. 

(a) The medical exam done at the employment time, the adaptation medical exam, the periodical medical exam and the return to work medical exam will be performed in conformity with the legal specification available.


(b) In special cases, strongly sustained by scientific proofs, the methodology will be annually modified to include new medical monitoring rules specific to the new professional risks.

From our point of view, the cardiovascular illnesses that have appeared or are aggravated by the occupational stress represents new professional risks specific to the top-level management of al the companies of institutions, along with employees from the following fields: special services - defense - public force, financial, health, transport, telecommunications - IT, education and public administration.

ORDER no. 803/12.11.2001 for the approval of the new indicators of exposure and / or the relevant biological effects in order to establish the specific response of the organism to risk factors that can lead to professional illnesses. But this also offers partial details about the unspecific response caused by stress and work overload to the employees.

The (unspecific) response of the organism: any change, be it reversible or irreversible, of the structure or the function of an organ, or system, or to the human body as a whole, that can be identified by the apparition of a symptom, syndrome or pathological entities for which there are scientific proofs of positive association with the exposure to an agent present in the workplace.

From our point of view the ocupational stress and the work overload generates unspecific responses that gradually, and unobserved lead to the apparition of chronic not transmissible diseases. From our point of view the chronic, un transmissible diseases that appear after the employment must be included on the list of the professional diseases because they were triggered or aggravated by the stress and work overload that took place at the workplace.

The annex attached to this order specifies that  The response indicators:

*are not specific

*can be used for as a start in the development of the health and security at the workplace strategy

*don't represent a criteria in the selections for the jobs that require special work conditions.

From our point of view all these response indicators constitute the unspecific response to stress and work overload. At high levels of tiredness, overload or stress the acute response of the organism can lead to work accidents and the presence of stress and overload at a lower level, but during long periods of time , can lead to the appearance of chronic un transmissible diseases. For this reasons we propose to research the including of stress and work overload at the workplace in the risk indicators calculations that is used by the assurances for work accidents and professional work diseases to calculate the amount paid by the employer for the employees assurance.

In the next paragraphs you can find an extract from this addendum that specifies the effects of stress and work overload have on the cardiovascular system:

-1- The following are possible body responses to stress caused by the mental overload and / or a cumulated sum of stressful conditions (stress, work overload beyond limits, noise, unfavorable physical work conditions):

- The increase of the instantaneous or average heart beat rate with more than 20 beats compared to the reference values
;

- The increase of the arterial systolic (and / or diastolic) pressure with more than 10mmHg during activity, compared to average values monitored during the reference period.

- The increase or the decrease of the heart beat rate variability compared to the reference values for more than 35% of the tested subjects.

- The presence of cardiac rhythm problems during the professional activity. Spikes of more than 1 mm under or over the ST-T segment median value.
 

- 2 - The following stress reactions are caused by the cognitive effort (mental or emotional) in work processes that involve strict temporal constrictions (very short and hard to beat time limits), uncontrollable factors, or activity that involves the public:

- The increase of the instantaneous or average heart beat rate with more than 40 beats compared to the reference values

- The increase of the arterial systolic and / or diastolic pressure with more than 40 mmHg during the activity, when compared to the reference values.

- 3 - The neuropsychological tiredness in activities that are monotonic - repetitive (like driving, command panel surveillance.. ) lead to :

- The increase or the decrease of the heart beat rate variability compared to the reference values for more than 35% of the tested subjects.

The arterial hypertension is one of the biggest health problems that affects 1 out of 4 adults. It is a major risk of premature death and for the apparition of important handicaps, and till it reaches its final stages it has the following negative effects: the diminishing of the intellectual performances, the diminishing of the memory or neural deterioration.

Other recent studies have demonstrated the way in which the un-managed stress cause the apparition and then the aggravation of the arterial hypertension that causes the diminishing of intellectual and physical performances, a decrease in the work productivity, extended medical leaves and, in its final stages, it can cause handicaps or even death among the employees (60% of the unexpected deaths by heart attack or cerebral accidents appear to persons that have not suffer of any cardiovascular disease)

 

In the last ten years the HearthMath institute demonstrated the link between the physical symptoms of stress and the diminishing efficiency of the employees at the workplace and offers techniques and instruments through which the employees can increase the efficiency of their activity simultaneously with the increase in the balance between work and family life.

Through the use of these techniques by the majority of the employees, the employers could observe an increase in the productivity and efficiency of their employees, the reduction of medical care costs, the decrease in absenteeism caused by medical conditions and an increase in the will of the employees to remain at that company, and not to go to other companies:
